Here's the lawyer answer -- it depends.  But it really does depend on your goals and on the complexity of the project.  


In general, I start simple.  I ask questions.  And then I listen.  And then I look.  I distill the information, identify pain points and work with the business to pressure test my perceptions and conclusions, revise if necessary, and partner to evaluate solutions.  Using design principles to innovate and improve the policies, products and processes, I focus on accurately assessing and understanding a company's unique situation rather than impulsively shoe-horning practices used in the past that "worked" at other places.
